PART ONE: Health status and behavior evaluation (10 pts., 1 & 2 (3 pts/each), 3 (4 pts.)
In this section, you will evaluate your screening and assessment process of yourself.
1. What is your health risk classification? Do you have any health concerns? Here, you may
write any type of injuries or pains as well as high blood pressure, etc. (health conditions).
2. What did you learn from the lifestyle questionnaire and the fantastic lifestyle
questionnaire?
3. Explain your health and fitness goals.
PART TWO: Testing results and interpretation (12 pts, 3 pts. each category)
In this section, for each component (below), tell me your results, category, and interpretation. Do
you think each test is a good representation of your fitness?
1. Body composition: we just did weight and waist to hip ratio. Discuss your BMI as well.
2. Cardiorespiratory fitness: include your heart ranges, your RHR, and the results of your
CR test (YMCA walk/run).
3. Muscular endurance: results of your push-up test
4. Balance results and category
PART THREE: Write your exercise program. (25 pts.- 5 pts. each question)
In this section, you will write an exercise program, for yourself. Based on the first two sections,
what will an exercise program look like for you?
1. What are the goals of your program? (i.e. I want to improve my aerobic fitness, this, is
what I will do; BE SPECIFIC
2. For both cardiorespiratory exercise (CR), write a program based on the FITT principle
a. Example: My CR program will consist of 2 days of running for 20” @ 70%
intensity, heart rate of 130
3. For resistance training (RT), write a program based on the FITT principle
a. Example: My RT training program will include 2 days of lifting, full body, 6
exercises (name them, order), 3 sets of 12 reps;
4. Summary: how do you plan to enhance your current program? Add more structure to it?
Create a new program? You can speak to your flexibility and balance. What do you do
currently? Will you incorporate this inro your program?
5. Explain what your learned in this class about exercise assessment and programming.
