  • Name: Linda M. 
  • Age: 46 
  • Occupation: Graphic Designer 
  • Chief Complaint: Right wrist pain and numbness interfering with work and daily activities 
  • Medical History: Raynaud’s Disease (diagnosed at age 30), mild hypertension 
  • Diagnosis: Suspected Carpal Tunnel Syndrome (CTS) – confirmed via nerve conduction study 
  • Onset: Gradual onset over the past 3 months; worsened over the last 3 weeks 
  • Symptoms: Tingling and numbness in the thumb, index, and middle finger; pain in the volar wrist; weakness with gripping 
  • Aggravating Factors: Prolonged computer use, repetitive wrist motions
